How to do End Screen for the form?

End Screen it is the part of the verification procedure for the signed forms.

You have two option to do End Screen when signing the form:

Printed

If you want to do End Screen with a printed version, you should:

• Print the PDF form that you received in the email
• Fill it in by hand
• Put your ID document in the orange frame
• Take a photo of both pages of the form

*Form cannot be filled in digitally and then printed.

Digital

If you want to do a digital End Screen, you should:

• Open the link that you received via email
• Fill in the form
• Take a photo of the opened form on the screen with your ID document
